Our Day Program

Open 9:30 am to 3:30 pm Monday - Thursday

Mountain Community Pathways Day Program and Offices

are located at 5120 Highway 73 in beautiful Evergreen, Colorado near the Evergreen Library.

It incorporates structured learning with fun activities

that provide individuals with social and recreational interests.  

The Day Program extends participants the opportunity to engage in

meaningful activities both in the community and at the day program facility.

The programs vary with respect to the nature of the needs of the participants,

including those with physical, medical and behavioral challenges. 

In addition, participants are encouraged to develop

independence through activities of daily living.

Our services provide a place outside the home for adults

with all types of disabilities to be active in the community,

socialize with their peers and receive needed health and personal care services.

Participants generally customize their individual dietary needs

by bringing their own lunches to day program

but also enjoy scheduled days where the group does their own cooking.

Activities encompass a wide variety of interests and choices designed

around a monthly theme while adapting to the individuals strengths.
